School Improvement Planning (Form 5)
Description:
This form is used to list the basic elements of your school improvement plan and will drive the following needs assessment forms. There can be more than one improvement strategy per student learning goal. There can also be more than one action plan per improvement strategy. The resources will fit into one of the following categories: materials, staff, scheduling, professional development, comprehensive reform model, or no cost.
Goal:
Define your school improvement plan.
Column Description:
Student Learning Goal – List student learning goals that you have identified during the school improvement planning process.
Improvement Strategy – List strategies that could help fund the student learning goal.
Action Plan – List detailed items needed to implement the improvement strategy.
Resources – Identify each resource as one of the following in the pull down menu: materials, staff, professional development, comprehensive reform model, or no cost.
